I have a similar problem, this is what I see in the access.log file of apache:
127.0.0.1 - - [18/Nov/2008:21:50:07 +0100] "GET /gerunda/index.php?option=com_recommendfriends&Itemid=26 HTTP/1.1" 500 -
It looks like I have the same problem... but in my case I can't open the page with the form, when I click over the menu item automatically appears a blank page.
It's a shame, the component looks great :-(
